Abstract

Medication errors are major health problems that increase the cost and threaten patient safety. Medication errors, seems to take place over several stages up to the preparation of pharmaceutical applications. Children may expose more frequent medication errors, because of lack of some pediatric drug forms and developmental characteristics such as body surface and weight. In addition, small children can not express their drug side effects makes it difficult to determine the cause of the damage from medication errors and mistakes. Therefore, prevention of medication errors in a pediatric population is of particular importance. There are two main reasons for the fundamental error; system factors and human factors. Further it is observed that the human factor is also the main reason is connected to the system. People are made to the nature of the error, the aim is to develop effective systems to minimize errors as possible. The prevention of errors, making the identification of errors and bugs are very important. Error notifications, in particular the reporting of errors made root cause analysis of near misses and allows to prevent similar mistakes. However, studies indicate whether enough of reported medication errors.
